By using our website, booking an appointment, or purchasing services from ClearID Laboratory LLC, you agree to the following Terms and Conditions.
1. Services
ClearID Laboratory LLC provides professional collection and support services, including drug testing collections, alcohol testing collections, DNA specimen collections, fingerprinting services, identity verification services, mobile and onsite services, alcohol monitoring technology device services, and other administrative and collection-related support services. All services are subject to availability, required documentation, and applicable laws and regulations.
2. Client Responsibilities
Clients are responsible for providing accurate and complete information, valid identification, completed forms, and any documents required for service. It is also the client’s responsibility to confirm that they are selecting the correct service required by their employer, court, agency, monitoring program, or other requesting party.
3. Payment
Payment is due at the time of booking or service unless otherwise agreed to in writing. Fees may include service charges, travel fees, mobile fees, after-hours fees, rescheduling fees, and administrative fees. All pricing is subject to change without notice unless confirmed in writing.
4. Refunds
All payments are generally non-refundable once an appointment has been scheduled, service preparation has begun, a technician has been dispatched, or services have been provided. Any refund request is subject to review and approval at the sole discretion of ClearID Laboratory LLC. Please review our Refund Policy for full details.
5. Appointments and Cancellations
Clients must arrive on time and be prepared with all required documents for their appointment. We reserve the right to reschedule, refuse, or cancel services due to lateness, missing documents, invalid identification, nonpayment, safety concerns, or other compliance-related issues.
6. No Guarantee of Outcome
ClearID Laboratory LLC does not guarantee any specific result, turnaround time, approval, acceptance, or outcome from any service provided. Final decisions and processing may depend on third-party laboratories, agencies, courts, employers, monitoring programs, or other authorized entities, all of which are outside of our control.
7. Alcohol Monitoring Technology Devices
If alcohol monitoring technology device services are offered, the client is responsible for the proper use, care, and compliance associated with the device while it is in their possession. ClearID Laboratory LLC is not responsible for violations, alerts, failed readings, missed tests, tampering results, or decisions made by any court, agency, monitoring authority, or third-party provider related to the use of the device.
8. Privacy
Your personal information may be collected, used, stored, and shared as necessary to provide services, verify identity, process documentation, coordinate with authorized third parties, and comply with legal and regulatory obligations. 9. Limitation of Liability
To the fullest extent permitted by law, ClearID Laboratory LLC shall not be liable for delays, third-party processing issues, missed deadlines, denied applications, employer or court decisions, monitoring program determinations, or any indirect, incidental, or consequential damages related to the use of our services.
10. Changes to Terms
We reserve the right to update or modify these Terms & Conditions at any time. Any changes become effective once they are posted on our website or otherwise made available to clients.
